A. Free Loan Service

Approved equipment and support items are loaned free of charge and remain the property of Citizens Neighborhood Network (CNN); they are never transferred to the recipient. Home-care equipment supports people recovering at home. Bereavement support may include chairs, oblong tables, and tents for a short period.
Approval is granted at CNN's discretion, based on genuine need, a completed application, and the availability of the requested items.
For home-care equipment, the usual loan period is 90 days from the date of issue, unless a shorter or longer period is agreed with CNN. Bereavement support items are short-term loans with a maximum duration of 10 days from issue. CNN and the recipient may agree an earlier return date before the items are issued.

B. Care of the Equipment

The recipient must use the equipment solely for its intended purpose, keep it clean, dry, and safely stored, and notify CNN immediately of any damage, fault, or malfunction.
The equipment must not be sold, hired out, pledged, modified, or transferred to any other person or household under any circumstances.
Where the equipment is damaged while on loan, CNN will arrange for repair or replacement. The cost will be recovered from the applicant and/or next of kin, except where the damage is due to normal wear and tear or a manufacturing defect.
Where the equipment is lost or stolen while on loan, the applicant and/or next of kin are liable for its full replacement cost.
The recipient must allow CNN reasonable access to inspect the equipment's condition on request.

C. Return of the Equipment

All items must be returned by the agreed due date, or as soon as they are no longer needed, whichever is sooner. For bereavement support items, the agreed short-term return date will be confirmed before issue and must be no later than 10 days after issue; home-care equipment is normally returned within 90 days.
Requests to extend a loan must be made to CNN before the agreed return date. Extensions are granted at CNN's discretion and confirmed in writing.
CNN may recover the equipment at any time — including before the loan period ends — if these terms are breached, or if the equipment is required urgently for another applicant with a documented emergency need.
The equipment must be returned clean and in reasonable working condition, fair wear and tear excepted.

D. Liability

Equipment is provided "as is," without warranty of any kind. CNN is not liable for any injury, loss, or damage arising from its use or condition, except to the extent required by law.
The applicant and next of kin jointly and severally indemnify CNN against any claims, costs, or damages arising from misuse, negligent handling, or unauthorized transfer of the equipment.

E. Your Information & Privacy

Information provided in this application, including next-of-kin and referee details, is used only to assess, process, and manage the application and loan.
Your data will not be sold or shared with third parties other than as needed to verify the application or recover equipment. Referees may be contacted to confirm the application is genuine. You may request access to, correction of, or deletion of your data at any time by contacting CNN.

F. Confirmation

By submitting this application, you confirm that all details provided — including next-of-kin and referee information — are true and complete. Providing false or misleading information may result in rejection of the application, immediate recovery of the equipment, and recovery of any associated costs from the applicant and/or next of kin.

G. Donations

This service is provided entirely free of charge. Donations are welcomed and help sustain CNN's home-care and bereavement support services.
